1. To tell you the truth, I decided to change my eating habits.
(1) truth is a noun, meaning "fact". Its adjective form is true. The phrase to tell (you) the truth means "to tell the truth" and is often placed at the beginning of a sentence as a parenthetical phrase without any element in the sentence.
